How to Use this Program

Patient Advocacy representatives will work with you to locate specific providers, confirm the provider's acceptance of new patients and/or provide you with a listing of several different providers in your immediate area that will meet your specific needs.

1. Call Patient Advocacy Representitives at 1-888-335-8222 to locate a participating provider in your area and confirm the provider's acceptance.
   
2. Present a copy of the Beech Street "Dear Provider" letter (enclosed in the pocket of this Member guide) to the provider's office.

When obtaining medical services, always remember to show your Heartland Alliance of America MD/Ancillary ID card to the provider. Important: Providers will not honor the network rates if your ID card is not presented at the time of service.
