How to Access Your Discounts:

1. To locate a participating provider, call 800-290-0523 or visit www.careington.com/heartland to access the online provider search.
2. When scheduling an appointment, member should inform the provider's office they are part of the Careington Dental Network.
3. Members must present their Careington ID card upon arrival to receive savings.
4. The provider will collect payment at the time of service.
   Member is responsible for paying the total bill, less the applicable savings, when service is provided.
