How to Access the Discount:

1. Please call 877-814-2461 between 8:00 a.m. and 8:30 p.m. (EST). Please reference Membership Guide (page 3) when scheduling in order to receive the discount. Payment will be required over the phone when the diagnostic test is scheduled.
   
   
2. Our service representatives will assist you in selecting a network provider that is convenient to your home or work.
   
   
3. Once you choose a facility, an appointment will be scheduled by conducting a unique “three-way” conference call with our service representative, the imaging center and you.
   
   
4. On the same call, you can ask any question you might have in regards to the test and what to expect, directions to the facility, etc.

Galaxy Health Network discounts are not available in HI, MT, ND, SD and WV.
