Private Health Care Systems (PHCS) is an independently-contracted network of over 525,000 physician locations. By using a PHCS provider, you can save an average from 20% to 45% on medical services such as routine office visits* and ancillary services.

How to Access Your Discounts:

1. To locate a participating physician or if you have questions about your physician visit, please call 1-888-335-8222. You may also find participating providers online at:
   www.careington.com/heartland.
   
2. When scheduling your appointment, please inform the provider’s office that you are a part of the PHCS Network.
3. At your visit, please present your PHCS Physician Savings card and the “Dear Doctor” letter included in your membership kit to the receptionist. Your card instructs the provider to call or fax Member Services for the negotiated rate agreed upon in their PHCS contract.
4. The physician will collect payment from you at the time of service. Please remember, you are responsible for paying the entire discounted fee at the time service is provided.

This plan does not meet the minimum creditable coverage requirements under M.G.L. c. 111M and 956 CMR 5.00. The plan provides discounts at certain health care providers for medical services. The range of discounts will vary depending on the type of provider and service. The plan does not make payments directly to the providers of medical services. Plan members are obligated to pay for all health care services but will receive a discount from those health care providers who have contracted with the discount medical plan organization.
